I always play on UV but have reason to suspect a recent run of Doom II may not be as watching BMD playthroughs of the same maps he has a few additional demons than I face. Is there a way to determine the difficulty level a map is played at after having "started a new game"?

In ZDoom-based ports you can open the console and type in "skill" to see the skill level you're currently playing on. '3' is UV.

3 is not UV, 3 is HMP.

0 = No Things in map (vanilla/chocolate only)
1-5 = The rest

I just tried it in zdoom and it definitely goes: 0 = ITYTD 1 = HNTR 2 = HMP 3 = UV 4 = NM
I don't know about other ports.

That's interesting, running doom from the command line, the -skill parameter is 1-5. I wonder why ZDoom changed it to 0-4
