In Doom 2 for me it's MAP25, it's just so bare bones and incredibly symmetric and empty, honestly saying, the most memorable thing about it is the title. In Ultimate Doom it's E4M4 Pretty much for the same reasons as MAP25 from Doom2, except less symmetric, but more blandly textured. In TNT it's MAP04 Because it's symmetric, boring and brands itself as an original map using the whole wormhole thing while MAP27 from Doom 2 already had that. In Plutonia it's MAP30, it's pretty much the last map I think of when I think of Plutonia as a whole, pretty much the same as MAP30 from Doom 2, except with a cyberdemon in front of the IOS and small Plutonia shenanigans to the sides.

I forget which side the switch is on in the final room of MAP04: The Focus every time. Every time.

Doom 2's "The Citadel" is a load of shit, for real. I did a recent run of Doom 2 and it was the only map I struggled with. Sure, some got a bit confusing because I was rusty—the open-ended nature of "Tricks and Traps" being a good example—but it's only in "The Citadel" that I truly got lost multiple times, to the point that I was able to find the secret yellow key, but not the actually out in the open blue key. It's a map that just has too many switches, teleporters and non-linear passages with few clues to aid the player. It's a real mess of a map, I hate it.
So I'd say the fact that I've played it numerous times and still can't recall where everything is, or how to even find the blue key, makes it forgettable.

I don't know why some people call forgettable maps that they hate for some reason or just maps that are mostly regarded as being bad (if anything those are the ones that are remembered the most), I've seen this also on DW a couple of times. Bloodfalls is truly the forgettable map of Doom 2, it isn't bad but it really lacks of memorable features. On Ultimate Doom E4M3-4-5 is a part that I struggle to recall. For TNT it's Baron's Den, and I agree that for Plutonia it's MAP30.

The Catacombs always sneaks up on me and I completely forget where to go and how to handle it.
Doesn't help that it's so late in that I virtually never play it.

If you're like me and you were 15 years old when DOOM 2 came out back in 1994, and you were totally blown away by the game and played it obsessively every chance you got, then literally none of the iwad levels are forgettable. They are all sacred. They are all burned into your memory forever. It's easy to point out flaws in those levels here in 2018, but in 1994 id Software could do no wrong in my young teenage mind.

Bloodfalls really lacks of memorable features. Not really, it's a rather innovative map, it invented the whole secret behind falling liquid, the X-shaped barrel placement, the cavern filled with torches, the subtle reference to the next map and the water cube exit. But the true problem is the layout, it's so bland, symmetric. The progression is also lame, go from room to room in a linear fashion.

Half of Plutonia is forgettable. That's why I prefer TNT and Doom 2, heck even Bloodfalls and Wormhole aren't as forgettable.

Slough of Despair. The name is cool, the map is boring as fuck. Oh cool...it's shaped like a hand...

It would be easy to say this as a snide joke, but I honest-to-god can't even remember the forgettable ones enough to tell you what they were. I get a vague sense of the shape of a room or two in my head but I can't for the life of me put a name on it. I guess E2M9 is one, since up until checking just now I had it mixed up with E4M9. And that one's at least noticeable for being the only real techbase in the entirety of episode 4.

Doom E2M3 is maybe the most forgettable map in my opinion. I don't like the idea of using the music of the intermission screen. Also , the map is incredibly flat and you must take a long path in order to reach the exit.
